PT-141 Bremelanotide for Men

PT-141, also known as Bremelanotide, is a synthetic peptide that is used primarily as a treatment for sexual dysfunction. It is a derivative of the peptide hormone melanocortin, which is involved in regulating various physiological functions including sexual behavior.

PT-141 works by activating the melanocortin receptors in the brain, which in turn increases sexual arousal and desire. It is primarily used to treat sexual dysfunction in both men and women, including erectile dysfunction and low libido.

PT-141 works in the brain, while Cialis (tadalafil) and Viagra (sildenafil) work by increasing blood flow to the penis. Effects of PT-141 typically last up to 36 hours.

Benefits of PT141 include:

  • Increase in libido levels
  • Elevated degrees of sexual gratification
  • Enhanced vigor and sexual desire
  • Strengthened penile erections

Dosing

Reconstitute with 10mLs of Bacteriostatic water then inject 25-50 units (0.5-1mg) SC about 45 minutes prior to sexual activity. If desired results are not achieved after 1 hour, inject 12.5 units (0.25mg) until desired results are achieved. ***DO NOT EXCEED 100 UNITS (2mg) IN 72 hours**

PT-141 (Bremelanotide) Clinical Information

How PT-141 Works

PT-141 (bremelanotide) is a melanocortin receptor agonist. Unlike PDE5 inhibitors (sildenafil, tadalafil), which act on blood flow, PT-141 acts in the central nervous system — specifically on melanocortin receptors (MC3R and MC4R) — to enhance sexual desire and arousal. It is administered by subcutaneous injection, typically 45 minutes before anticipated sexual activity.

Indications

  • FDA-approved: Hypoactive Sexual Desire Disorder (HSDD) in premenopausal women (under the brand name Vyleesi)
  • Off-label clinical use: Low libido or sexual response in men and post-menopausal women, under physician supervision

Contraindications

  • Uncontrolled hypertension (PT-141 can transiently raise blood pressure)
  • Known cardiovascular disease — discuss carefully with your provider
  • Pregnancy or breastfeeding
  • Known allergy to bremelanotide

Use with Caution If You Have

  • Resistant hypertension
  • Recent cardiovascular events
  • Medications that affect blood pressure (alpha-blockers, antihypertensives)
  • History of melanoma (theoretical concern given melanocortin receptor activity)

Common Side Effects

  • Nausea (most common; especially with first 1–2 doses)
  • Facial flushing
  • Headache
  • Transient increase in blood pressure (averages ~6 mmHg systolic for 8–10 hours)
  • Injection-site reactions
  • Darkening of the skin (focal hyperpigmentation; rare and reversible)

Dosing Notes

  • Typical starting dose: 1.75 mg subcutaneously, 45 minutes before activity
  • Maximum: 1 dose per 24 hours, 8 doses per month
  • Begin with a smaller test dose if nausea is a concern
